Times of Refreshing Wellness Retreat and Coaching

This application should be used for the wellness retreat and wellness coaching services of the ministry.

You can apply three ways:

  1. Online application
  2. Download a Microsoft Word or pdf file
  3. Request an application to be sent by mail. Contact us by phone, email or fax. Send the application back to us.

Click the link below to be taken to our secure online application! There are about 30 queststions relating to your wellness and life-style habits and it may take up to 30 minutes to complete. This is the fastest way to apply, however, it is recommended that you have information handy that relates to your wellness history such as any medication you may be taking. If you get interrupted for a significant period of time, you will be logged out and will have to begin the application again.

Wellness Retreat and Coaching Application

Both our wellness application in Microsoft Word and our wellness application in Adobe PDF, can be downloaded. You can then fill either of them out on your computer or print them out and fill out the application by hand.

If you fill out the application on the computer you can SAVE THE FILE, email the application back to us as an attachment or you can print it in preparation to mail or fax it back to us. Our email address and fax number is located in the header of every page.

If you choose to download the application in Microsoft Word, you will be able to type your answers in the document.

If you choose to download a pdf, the document will open in Adobe pdf reader with the "Typewriter tool" enabled.

You will see a purple bar at the top. Click the typewriter tool shown above and then click wherever you want to type and begin typing your answers. If you need to check multiple choice answers, simply click where you want to make a selection and type an "X" to make your selection.
